Chrysalis Application

The Chrysalis Flight is open to Christians of any denomination who are seeking a closer relationship with the Lord.
Do You Have a Sponsor?
In order to attend an Chrysalis Flight, you must have a sponsor - someone who has attended a previous flight or Walk to Emmaus and will serve as your guide before and after the weekend. If you do not know anyone who could serve as your sponsor, please email or call a member of the Cincinnati Chrysalis Board. A listing of the Board is available on this site.
Cost
The cost of attending the Chrysalis Flight is $75; this covers all food, accommodations, supplies, etc. used during the weekend. Scholarships are available for those who are in need. Please let your sponsor know if you are in need of a scholarship, and the fee will be covered for you.
Submitting an Application
To submit an application, please follow these few steps:
- Print out the application
There are 2 parts to the application - both of these are in the provided pdf file. Please print all the pages and continue to the next step.

Our applications require Adobe Acrobat Reader. It may be downloaded, free of charge, from Adobe's website.
- Complete the Catapiller's Application with your personal information.
- Hand your sponsor the following
- Completed Caterpiller's Application
- Blank Sponsor's Application
- A non-refundable deposit of $25. Please make checks payable to the Greater Cincinnati Chrysalis Community.
Your sponsor will complete the Sponsor's Application and send the entire application/deposit to the address contained on the application.
